七月十一日下午两点,吐鲁番市人民医院。
神经内科在三楼。
走廊很长,两边都是病房,中间放着几张塑料椅。
阿里木坐在从头数第三张上。
他还穿着昨天晚上那身衣服,裤腿上有土。
那是三号房门口的土。
马可上楼的时候,他站起来。
站起来以后他没有说话。
马可问:
“在哪个房?”
阿里木指了指走廊尽头。
他说:
“三〇七。”
这三个数字他说了两遍。
第二遍是说给自己听的。
马可问:
“现在什么情况?”
阿里木说:
“脑出血。”
他说:
“右边不能动。”
他说:
“说不了话。”
马可问:
“能听见吗?”
阿里木说:
“能。”
他说这个字的时候声音有点变。
他说:
“大夫说他能听见,也能懂。”
他说:
“就是说不出来。”

“夫二仪之内,阴阳之中,唯人最贵。人者,禀气含灵,恃我以生。故经曰:天地之中,以人为贵。……人命至重,有贵千金,一方济之,德逾于此,故以为名也。”

“凡大医治病,……省病诊疾,至意深心,详察形候,纤毫勿失,处判针药,无得参差。虽曰病宜速救,要须临事不惑,唯当审谛覃思,不得于性命之上,率尔自逞俊快,邀射名誉,甚不仁矣。”
“胆欲大而心欲小,智欲圆而行欲方。”
*English Mirror: Within the two principles, between yin and yang, man alone is most precious. Man receives breath and holds spirit, and depends upon this for life. Therefore the classic says: in the midst of heaven and earth, man is the precious thing. … A human life is of the utmost weight; it is more precious than a thousand pieces of gold, and a single prescription that saves it has a virtue beyond even that — and so I have taken it for the title. — Whenever a great physician treats an illness he must settle his spirit and fix his will, wanting nothing and seeking nothing, and first bring forth a heart of great compassion and pity, vowing to save universally the sufferings of all who hold spirit. If any come in sickness and distress to seek help, he must not ask whether they are noble or base, rich or poor, old or young, beautiful or ugly, enemy or kin, close friend or stranger, Chinese or foreign, foolish or wise — he must treat them all alike, as though each were his nearest relation. 那天下午,马可先去交了钱。
押金八千。
窗口那个人问他跟病人什么关系。
马可想了一下,说:朋友。
交完他站在缴费窗口前面,把手机银行打开看了一眼。
三万五千四百,变成两万七千四百。
这个动作他做完,自己知道不该做。
可是他还是做了。
他把手机收起来,回三楼。
阿里木在门口等他。
阿里木说:
“这个钱我们家出。”
马可说:
“先不说这个。”
阿里木说:
“我二哥后天到。”
他说:
“他在广州,明天的机票。”
马可问:
“你姐呢?”
阿里木说:
“她在乌鲁木齐,今天晚上到。”
马可点了点头。
他没有再说钱的事。
因为这个时候说钱,说什么都不对。
可是不说,钱也在那儿。
马可在走廊上算了一遍。
押金八千是三天。
脑出血的病人在神经内科,一天一千上下。
要是住二十天,两万。
要是转ICU,一天三四千。
这些数他一个也不该算。
可是他算了。
算完他又算了另外一头。
七月十八号前后是那三座房下架的日子。
下完架要收货,收货要付款。
去年那三座房出了一千三百斤,一万九千七百六十。
这笔钱今年谁付?
罗代办今年那一笔已经做完了。
马可自己卡上还剩两万七千四。
两万七千四,减去货款,减去这个走廊上每天要交的钱。
他把这一段在心里算到底,停了。
因为算到底那个数是负的。
那天下午三点半,马可进了三〇七。
四张床,靠窗那一张。
老人躺着,头偏向窗户。
右边那只手放在被子上,手指微微弯着,不动。
左手在被子下面。
马可站在床边。
阿里木在他旁边,说了两句维语。
老人的眼睛动了一下,转过来。
他看见了马可。
他的嘴张了张。
没有声音。
张了三次。
第三次以后他停了。
眼睛还看着马可。
阿里木在旁边说:
“他认得你。”
马可在床边站了一会儿。
他不知道该说什么。
这三年他跟这个老人说过的每一句话,都是关于房、关于孔、关于天数的。
没有一句是别的。
他站在那儿想了一遍,发现自己连一句寒暄都不会说。
最后他说的是:
“大叔,三座房我看着。”
阿里木翻。
老人的眼睛动了一下。
眼皮眨了两次。
阿里木说:
“他知道了。”
马可又说了一句:
“您那六个字,我还没有弄明白。”
这一句阿里木翻完,老人看着他,眼睛没有动。
过了大概五秒,他的嘴又张了一次。
还是没有声音。
马可说:
“您别急。”
他说:
“我明天去看。”
说完他退到床尾。
退的时候他知道自己刚才那一句是废话。
「您别急」这三个字,是说给自己听的。
段三·事件主体
孙思邈是那天晚上出现的。
九点多,探视时间过了。
马可在三楼走廊那排椅子上坐着。
阿里木回村里拿东西去了。
走廊尽头的灯是暗的,只留了两盏应急灯。
有一间病房里有人在低声说话,说的是维语。
有一个人从那头走过来。
很老。
头发和胡子都是白的,梳得很整齐。
穿的不是官服,是道士那种袍子,青灰色。
他背上背着一个布囊。
囊口露出几根草。
他走到马可旁边那张椅子,坐下了。
坐下以后他没有立刻说话。
过了一会儿他说了第一句:
“你刚才在缴费的地方,看了一眼手机。”
马可说:
“先生。”
那个人说:
“孙思邈。”
那天晚上,孙思邈先说了他那本书的名字。
他说:
“我写过两本,都叫千金。”
马可说:
“《千金要方》。”
孙思邈说:
“还有一本《千金翼方》。”
马可问:
“为什么叫千金?”
孙思邈说:
“序里我写了。”
他说:
“人命至重,有贵千金。”
他说:
“一方济之,德逾于此。”
他抬起头。
他说:
“一条人命,比千金还重。”
他说:
“一个方子救了它,那个方子比千金更重。”
他把布囊从背上取下来,放在腿上。
他说:
“所以我拿这两个字做书名。”
马可问了一句。
他说:
“先生,那千金是多少?”
孙思邈看了他一眼。
他说:
“你问的是数?”
马可说:
“是。”
他说:
“我今天下午交了八千。”
他说:
“交完我看了一眼卡上还剩多少。”
他说:
“我知道不该看。”
孙思邈说:
“该看。”
这两个字他说得很快。
马可停了一下。
孙思邈说:
“我那篇《大医精诚》里写:不得瞻前顾后,自虑吉凶,护惜身命。”
他说:
“很多人把这一句读成:不许算。”
他抬起头。
他说了一句,马可当时坐直了:
“我说的是不许因为算了就不去。”
他说:
“不是不许算。”
孙思邈接着说了下去。
他说:
“我行医六十年。”
他说:
“每一次出诊,路多远,天多冷,我心里都有数。”
他说:
“有几次我走了三天。”
他说:
“三天里我算过:这三天我在山里能采多少药。”
马可说:
“先生也算这个?”
孙思邈说:
“算。”
他说:
“算完还是去。”
他把布囊的口收紧了一点。
他说:
“一个不算的人,走三天到了,心里是空的。”
他说:
“他觉得自己了不起。”
他说:
“觉得自己了不起的人,看病不细。”
他抬起头。
他说了一句,马可当天在本子上抄了三遍:
“算清楚了还去的那个人,到了以后心里是稳的。”
马可问了一句。
他说:
“先生,那算完不去的呢?”
孙思邈说:
“那就是不去。”
他答得很平常,一点也没有责备的意思。
他说:
“我那篇里骂的是这一种。”
他说:
“我用了四个字:含灵巨贼。”
马可说:
“这四个字很重。”
孙思邈说:
“重。”
他说:
“我写的时候也知道重。”
他说:
“可是这一行里,轻话没有用。”
他把那个布囊换了一只手拿。
他说:
“一个大夫算完不去,没有人会知道。”
他说:
“他不去,那家人就找别的大夫。”
他说:
“找不着,人死了,也没有人怪他。”
他抬起头。
他说:
“这件事从头到尾,只有他自己知道。”
他说了一句,马可当天在本子上抄了两遍:
“只有他自己知道的事,得用最重的话来说。”
那天晚上,马可跟孙思邈说了那六个字。
三号房那一串,今年别换。
说完他把三种读法也说了。
第一种:别换,就是留着去年阿里木挂的那一串。
第二种:别换,是今年三号房不挂新的。
第三种:别换,是三号房今年用二号房那一串。
孙思邈听完,问了一句:
“这三种,哪一种最省事?”
马可想了一下。
他说:
“第一种。”
他说:
“第一种什么也不用做。”
孙思邈说:
“那就先把第一种放下。”
马可问:
“为什么?”
孙思邈说:
“一个人半边身子不能动,嘴里说不出话。”
他说:
“他攒了一口气,说了六个字。”
他抬起头。
他说:
“这六个字要是「什么也不用做」,他不会说。”
马可在椅子上坐着,把这一层想了一遍。
孙思邈又说了一段。
他说:
“我那篇里还有八个字。”
他说:
“详察形候,纤毫勿失。”
马可问:
“形候是什么?”
孙思邈说:
“看得见的那些。”
他说:
“脸色,舌头,脉,出汗不出汗,什么时候疼。”
他说:
“病在里头,看不见。”
他说:
“能看见的只有形候。”
他把布囊放回背上。
他说:
“你现在的病人是那六个字。”
他说:
“六个字是里头。”
他抬起头。
他说了一句,马可第二天一早就回了那一排房:
“你去看形候。”
马可问了最后一句。
他说:
“先生,形候看完了,还是不知道呢?”
孙思邈说:
“那就是不知道。”
他说:
“我一辈子看过很多不知道的。”
他说:
“《千金方》里有很多方子,后头我加了一句:未试。”
马可停了一下。
这两个字他见过。
二〇二九年他在袋子上印过。
孙思邈说:
“一本方书里,有几个方子标着「未试」,看的人反而敢用别的。”
他说:
“通篇都不标,看的人一个也不敢用。”
他站起来,把布囊背好。
他说了走之前那一句:
“不知道要写在方子旁边。”
他说:
“写在旁边,那个方子还是能用的。”
他说:
“藏起来,整本书就都不能用了。”
段四·钩子结尾
七月十二日上午,马可回了那一排房。
从县城过去四十分钟。
三座房都关着。
他先进的是三号。
门框内侧那一串在。
是去年阿里木挂的那一串。
低两拃。
马可伸手捏了一下。
很硬。
比一号那一串硬。
他把这个记下来。
记的时候他忽然想起一件事。
这个「比一号那一串硬」,他去年五月也记过。
去年记的时候他写的是:三号那一串挂低了两拃,是阿里木挂的,挑的是他自己觉得好的。
这句话里有一个跳跃。
从「挂低了」跳到「挑错了」。
这两件事之间,去年他没有验过。
验的办法他今天想到了:
去年三座房下架的日子,阿里木说过。
一号七月九,二号七月十一,三号七月十四。
三号比一号晚五天。
要是三号那一串太严,下架就会晚。
这一条对上了。
可是还有另外一种可能。
三号房本身就慢。
要是三号房本身就比一号冷,那它本来就该晚下。
这两种可能,去年的数里有没有?
马可站在三号房门口,想起了那六支温度计。
阿里木记了一百零一天,一天两次。
那一份他到今天还没有打开过。
然后他做了一件他以前没有做过的事。
他把一号、二号、三号那三串,全取下来,摆在门口的地上。
三串并排。
从左到右:一号,二号,三号。
摆完他蹲下去看。
看了大概十分钟。
看出来三样。
看的时候他用的是孙思邈那八个字。
详察形候,纤毫勿失。
他先看整串,再看单粒,再看串跟串之间。
第一样:三号那一串的果子,个头比另外两串小。
第二样:三号那一串的颜色比另外两串深。
第三样:三号那一串上,果子的间距比另外两串密。
这三样他一样也说不清是什么意思。
说不清,可是他没有停。
他把这三样又各分了两头。
果子小:可能是那一年树上就小,也可能是晒缩得多。
颜色深:可能是晒得久,也可能是那一座房光进得多。
挂得密:可能是挂的人手法,也可能是那一年果子多。
六种可能,一种也验不了。
因为这三串都是去年的,去年的树、去年的天、去年的手,全过去了。
马可在本子上写了一行:
「看得见的东西,不一定说得出原因。」
他停了一下,写完下半句:
「可是不看,连有几种可能都不知道。」
他把它们写下来,一样写一行。
写完他把三串挂回去。
挂的时候他一串一串对着原来那个钉子,没有换位置。
因为老人说的是「别换」。
这两个字他现在只能按最窄的意思守着:一串也不动。
挂的时候他量了一下高度。
一号一米五四,二号一米五三,三号一米二九。
这个数他去年五月就知道。
今年他量了第二遍。
量完他在本子上写了一行。
他写的是:这个数两年没有变。
写完他又加了一句:
「两年没变,说明这两年三号房那一串都是同一个高度。」
「同一个高度,说明这两年都是阿里木挂的。」
这一层他去年没有想到。
去年他以为那是一次意外。
是老人腰不好,那一天让儿子代了一次。
今年那一串还在一米二九。
那就不是一次。
去年封房那天,老人是不是又让儿子挂了?
还是那一串根本就没有换过?
这两样也分不出来。
因为马可去年十一月没有在场。
那天中午,马可去了第十二座。
他找热合曼江,问了一句。
问的是:
“您门框上挂串吗?”
热合曼江说:
“挂。”
马可问:
“几时开始挂的?”
热合曼江说了一句,阿里木不在,是他侄子翻的。
翻过来是:
“去年六月看见书以后。”
马可问:
“在这以前呢?”
热合曼江说:
“不挂。”
马可问:
“那以前怎么比?”
热合曼江说了一段。
侄子翻:
“他说,以前不比。”
他说:
“他说,咬了就知道,不用比。”
马可停住了。
他问:
“那阿不都热合曼为什么要挂?”
热合曼江看了他一眼。
他说了一句。
侄子翻的时候顿了一下:
“他说,那是你让他挂的。”
马可在那个院子门口站了很久。
这一句他不能不认。
那一串是他二〇三〇年三月在常那儿看见的。
那一年十一月封房,他跟阿不都热合曼说:留一串。
老人说了一个字:好。
从那一年起,那三座房门框上就挂着三串。
挂了两年。
这两年,老人自己咬不咬?
马可这两年从来没有问过这一句。
因为他默认:老人咬,然后拿那一串对。
今天他知道,也可能是另外一种。
老人咬,那一串挂在那儿给他看。
给谁看?
给来了的人看。
马可在本子上写了一行:
「那三串,可能一天也没有用过。」
他停了一下,写完下半句:
「挂着,是因为我要看。」
写完这两行,马可又往下写了一层。
他写的是:那要是这三串真的一天也没有用过,它们有没有用?
答案是有。
去年五月罗代办一眼看出三号那一串挂低了两拃。
那一眼看出来的不是货。
是「这一座不是同一个人管的」。
这个信息是真的,而且要紧。
所以那三串确实有用。
只不过它的用处,不是马可以为的那一个。
它不是老人的尺。
它是一个外人从门口就能读到的东西。
马可在本子上写了一行:
「我装了一个仪表,以为它是给做的人看的。」
他停了一下,写完下半句:
「两年下来,读它的只有我。」
再下面一行他写完把笔停住了:
「那它就不是仪表,是我的眼睛的延长。」
七月十四日下午,医院。
阿里木的二哥到了,姐姐也到了。
病房里人多,马可在走廊等。
三点多阿里木出来叫他。
他说:
“我爸醒着。”
他说:
“他刚才一直在看门口。”
马可进去。
老人躺着,眼睛睁着。
看见马可,眼睛动了一下。
马可走到床边,蹲下来,让自己的脸跟他齐平。
他说了一句:
“大叔,我去看了。”
阿里木翻。
马可说:
“三串我都取下来看过。”
他说:
“三号那一串,果子小,颜色深,挂得密。”
他说:
“这三样我不知道是什么意思。”
他说完等着。
老人看着他。
过了一会儿,被子动了一下。
是左手。
那只手从被子底下伸出来,很慢。
手伸到被面上,停了一下。
然后食指动了。
他在被面上画。
画得很慢,画了三次。
每一次都是同一个东西。
一个圈。
马可蹲在床边,看着那只手指在被面上转。
转完手停在那儿,没有收回去。
阿里木在旁边问了一句维语。
老人没有反应。
他还在看马可。
马可伸手,把自己的手放在那只手边上,没有碰。
他说了一句:
“我知道了。”
说完他自己的声音有点抖。
因为这个圈是他教的。
六月四号那天上午,他在院子里跟这个老人解释过两句。
这一条我问了,您答不上来,所以这一条不算数。
不是您做得不对。
当时他以为他给出去的是一个说明。
今天他知道,他给出去的是这个人现在唯一能用的一个字。
马可从三〇七出来,在走廊上站了很久。
阿里木跟出来了。
他问:
“那个圈是什么意思?”
马可说:
“是我教他的。”
阿里木说:
“六月那天我在旁边。”
他说:
“你说的是:这一条不算数,不是您做得不对。”
马可说:
“是。”
阿里木问:
“那他现在画这个,是说他也不知道?”
马可说:
“是。”
阿里木在走廊上站着,没有说话。
过了一会儿他说:
“那六个字是他自己说的。”
他说:
“他自己说的话,他自己不知道是什么意思?”
马可说:
“他知道。”
他说:
“他不知道的是我刚才问的那三样。”
他说:
“果子小,颜色深,挂得密。”
他说:

阿里木停了一下。
他说:
“那我们问错了。”
马可说:

过了一会儿阿里木问了一句。
他说:
“那该问什么?”
马可说:
“我不知道。”

“我得先知道他是怎么看的。”
阿里木说:
“他从来不说这个。”
马可说:
“我知道。”
阿里木说:
“我小时候问过。”
马可问:
“他怎么说?”
阿里木说:
“他说你多咬几年就知道了。”
马可站在走廊的窗户边上。
外面是医院的院子,有几棵树。
他说:
“这句话不是敷衍。”
阿里木说:
“我以前觉得是。”
马可说:
“现在呢?”
阿里木想了一会儿。
他说:
“现在我觉得他是真的说不出来。”
他说:
“我记了两年表。”
他说:
“我现在下架的时候,也是先咬一口。”
他说:
“咬完我知道行不行。”
他停了一下。
他说:
“你现在问我为什么,我也答不上来。”
马可看着他。
他说:
“你才十九。”
阿里木说:
“是。”
他说:
“所以我说不上来这件事,跟年纪没关系。”

苏东坡当夜记下的:这一回他看见了什么
Eleventh of July, two in the afternoon, Turpan City People's Hospital. Neurology is on the third floor. A long corridor, wards down both sides, a few plastic chairs in the middle. Alim was on the third chair from the end, still in yesterday's clothes with dirt on the trouser legs. He stood up and didn't say anything.
Which room? A gesture at the far end. 307. What's the situation? Cerebral haemorrhage. The right side won't move. He can't speak. Can he hear? He can — and his voice changed on the word. The doctor says he can hear and he understands. He just can't get it out.
I paid the deposit first. Eight thousand. Standing at the window afterwards I opened the banking app and looked. Thirty-five thousand four hundred became twenty-seven thousand four hundred. I knew while doing it that I shouldn't. I did it anyway. Alim, at the door: we'll pay that. Not now. My second brother lands the day after tomorrow — he's in Guangzhou, flight tomorrow. My sister's coming from Ürümqi tonight. I didn't raise the money again. At that moment anything you say about money is wrong.
307 at half past three. Four beds; his is by the window. He was lying with his head turned to the glass. The right hand on the coverlet, fingers slightly curled, not moving. Alim said something in Uyghur. His eyes moved and came round. He saw me. His mouth opened. No sound. It opened three times, and after the third he stopped, still looking at me. Alim: he knows you.
Sun Simiao came at nine, after visiting hours, down the dark end of the corridor — very old, white hair and beard combed neatly, not an official's robe but a Daoist one, blue-grey, a cloth bag on his back with the ends of some herbs showing at the mouth. He sat down on the next chair and said nothing for a while. Then: at the payment window just now, you looked at your phone. Sir. Sun Simiao.
On the title: I wrote two books, both called Qianjin — a thousand gold. Essential Prescriptions Worth a Thousand Gold. And the Supplement. Why that name? It is in the preface. A human life is of the utmost weight; it is more precious than a thousand pieces of gold. And a single prescription that saves it has a virtue beyond even that. And: so I took those two characters for the title.
I asked how much a thousand gold is. He looked at me. You're asking for the figure? Yes. And: I paid eight thousand this afternoon and then looked at what was left. I know I shouldn't have looked. — You should have looked. Immediately. I stopped. And then: in my Great Physician's Sincerity I wrote: he must not look before and behind, calculating his own fortune or misfortune, cherishing and protecting his own life. And: a great many people read that as: you may not calculate. And, and I sat up: what I said was that you may not use the calculation as a reason not to go. And: not that you may not calculate.
And the elaboration, which is the part nobody quotes: I practised for sixty years. Every call I went out on, I knew how far the road was and how cold it would be. And: a few times I walked three days. And: during those three days I worked out how much medicine I could have gathered in the hills instead. You calculated that too? I did. And I went anyway. Drawing the mouth of the bag tighter: a man who does not calculate arrives after three days with nothing inside him. He thinks himself admirable. And: a man who thinks himself admirable does not examine carefully. And, three times into the book: the man who has done the sum and goes anyway arrives steady.
Then the six characters, and the three readings: (i) leave last year's string up; (ii) hang none this year in house three; (iii) use house two's string for house three this year. Which of the three takes the least doing? The first — the first requires nothing at all. Then set the first one aside. Why? A man whose half body will not move and whose mouth will not work gathered one breath and produced six characters. And: if those six characters meant "do nothing," he would not have said them.
And the eight characters from the same essay: observe the signs in detail and lose nothing so fine as a hair. What are the signs? The things you can see. Colour of the face, the tongue, the pulse, whether he sweats, when the pain comes. And: the illness is inside and cannot be seen. All you can see is the signs. Shouldering the bag: your patient at present is those six characters. The six characters are the inside. And, and I was back at the row by first light: go and look at the signs. — M.

Sun Simiao's correction to the famous line — he must not look before and behind, calculating his own fortune — is that it forbids using the calculation as a reason not to go, not the calculation itself. 2. The six characters: 三号房那一串,今年别换. Three readings: (i) leave last year's up; (ii) hang none this year; (iii) use house two's for house three. Sun Simiao's elimination is not textual. Which takes the least doing? (i). Then set (i) aside — a man in that state gathered one breath for six characters. If they meant "do nothing," he would not have spent the breath.
3. What I found at the row, 12 July — the "signs," and none of them is conclusive. I took all three strings down and laid them on the ground side by side, houses one, two, three, for the first time in two years. — House three's fruit is smaller than the other two. — House three's is darker. — House three's are spaced closer on the string. Heights re-measured: 1.54 m, 1.53 m, 1.29 m — the same as May 2030. I cannot say what any of the three observations means. I wrote them down anyway, one line each.
4. And then Rehmanjan at house twelve, who took the ground out from under two years of my notes. — Do you hang a string? Yes. Since when? Since I saw the book last June. And before that? We didn't. Then how did you compare? We didn't compare. You bite it and you know. There's no need to compare. — Then why does Abdul Rahman hang one? He looked at me, and the nephew hesitated before rendering it: "Because you told him to."
5. Which I cannot dispute. I saw the string in Chang's doorway in March 2030. At the closing in November 2030 I asked Abdul Rahman to keep one back. He said one word: 好. There have been three strings on those doorframes for two seasons. In two seasons I have never once asked whether he uses them. I assumed: he bites, then checks against the string. The other possibility is that he bites, and the string hangs there to be looked at. Looked at by whom? By the person who comes. In the book: those three strings may never have been used once. They hang because I look. Chang's string is thirty-one years old and regenerates itself out of the accepted batch; it is an instrument. Mine is two years old and was installed on my instruction; it may be a display. The two are indistinguishable from the doorway, and I have been treating the second as evidence of the first.
6. 14 July, 307. Both siblings had arrived. He was awake and had been watching the door. I crouched to his eye level and reported: I went and looked. I took all three down. House three's are smaller, darker, closer together. I don't know what those three things mean. And then waited. The coverlet moved. The left hand came out, slowly, rested on the cover — and the index finger drew. Slowly, three times, the same shape each time. A circle. And then the hand stopped there and did not go back. Alim asked him something in Uyghur; no response; he was still looking at me. I put my hand on the cover beside his without touching it and said: I understand. My voice was not steady.
7. Because the circle is mine. On the morning of 4 June, in that courtyard, I explained it to him in two sentences: this line I asked, you couldn't answer, so it doesn't count — and it does not mean you did anything wrong. I thought I was handing over an explanation. What I handed over was the one mark this man now has any use for. He cannot speak, he cannot write, he has never read a character in his life — and he has exactly one piece of shared notation with me, and he used it to tell me he does not know either.
8. Which leaves the position, stated plainly. The bite exists in four men in two prefectures, three of them over sixty. The one I know best can no longer say anything except "I can't answer." The six characters remain undecoded. The July take-down for those three houses is in about a week and somebody has to decide.

"What I said was that you may not use the calculation as a reason not to go. Not that you may not calculate."
The two books are named for a line in the preface of the first. A human life is of the utmost weight; it is more precious than a thousand pieces of gold — and a single prescription that saves it has a virtue beyond even that.
The essay that goes with them is the one every physician in the tradition has had to answer to since. It requires that a doctor treat the noble and the base, the rich and the poor, the beautiful and the ugly, enemies and kin, strangers and friends, foreigners and Chinese, the foolish and the wise all alike, as though each were his nearest relation — and then adds the harder clause: nor may he look before and behind, calculating his own fortune or misfortune, cherishing and protecting his own life.
Told by a man that he had paid a hospital deposit that afternoon and then immediately checked his own balance, and had known while doing it that he should not have, the author of that clause said, without pausing: you should have looked.
And then he explained what the clause forbids, which is narrower and more usable than the way it is usually read. Sixty years of practice; every journey costed before he set out; three-day walks on which he had worked out exactly how much medicine he could have gathered in the hills instead — and went.
A man who does not calculate arrives after three days with nothing inside him. He thinks himself admirable. And a man who thinks himself admirable does not examine carefully.
The man who has done the sum and goes anyway arrives steady.
The same essay contains the instruction that governs everything after the arrival: observe the signs in detail and lose nothing so fine as a hair. Asked what the signs are, he was concrete — the colour of a face, the tongue, the pulse, whether the man sweats, when the pain comes — and then stated the reason the list matters.
The illness is inside and cannot be seen. All you can see is the signs.
